Restorative Care Pathway – Supporting eligible older Australians with short-term, goal-focused care designed to improve or maintain their abilities, rebuild confidence, and support independence.
Ongoing Care Coordination and Support – Helping organise services, communicate with relevant providers, and ensure care continues to meet your changing needs.
